Greetings!
By way of introduction, we are from R.S. Bernaldo and Associates (RSBA), a professional firm offering a wide range of assurance and non-assurance services across the country. We are outsourced by UHS Essential Health Philippines, Inc. (UHS) to ensure that all associates issue official receipts for the commissions received and that the receipts are in compliance with government laws and regulations.
Please take note that the scope of our services will not include the following:
- Any application for business registration of the associate with the Department of Trade and Industry (DTI) as sole proprietorship.
- Any application for registration with the Bureau of Internal Revenue (BIR) of the official receipts of the associates.
- Consultation of the associates regarding legal, taxation and accounting matters.
- Preparation of BIR form 2307 (Certificate of Creditable Tax Withheld At Source)
Enumerated below are the steps that the associate must do for each commission week:
Step 1. Each associate must provide soft copies of business registration certificates to RSBA team.*
Step 2. On each commission payout date, each associate to send us a soft copy of duly accomplished official receipt.**
Step 3. Hard copies of the official receipts must be forwarded to the RSBA within one (1) week after the commission was received. Our firm’s address is as follows:

As agreed with UHS, all delivery fees and charges will be handled by the associate.
Step 4. Steps 2 and 3 will be repeated for the next commission payout.
